This form is for those individuals who wish to remain anonymous and/or want to report something, but don't exactly know what they should do. Participants and guardians should continue to communicate with The Residency directly via email or phone, if they are safe/comfortable doing so.

This is a proactive way to ensure that lines of communication are open and a way exists for students to report incidents and maintain anonymity.
